    Questions

What does a parent need for a dependent care claim?

Requirements vary. Many programs ask for the provider's name, address, tax ID and the amounts paid. Check your program or tax authority.

Can I list several weeks on one receipt?

Yes. List each week or date range as its own line with the amount paid.
